Four Phases of Recovery
Phase One

Assessment & Stabilization

Your journey begins with a comprehensive clinical assessment and a meeting with our clinical team within your first 24 hours. Together with your assigned therapist and case manager, you'll develop a personalized treatment plan that addresses both substance use and any co-occurring mental health conditions. This phase is about getting grounded — understanding where you are so we can map where you're going.

Phase Two

Skill Building & Curriculum

With your treatment plan in place, you'll move into the heart of the program. Daily group sessions build your toolkit — CBT helps you identify and reframe negative thought patterns, DBT teaches emotion regulation and distress tolerance, and ACT helps you develop psychological flexibility. Weekly individual therapy goes deeper into your personal story. Between sessions, psychoeducation groups give you the knowledge to understand what's happening in your brain and body during recovery.

Your daily rhythm settles into a structured pattern: morning groups followed by individual work, a shared lunch that builds community, then afternoon psychoeducation and case management before heading home at 3:30pm. It's intensive but sustainable — designed to fill your day with purpose without burning you out.

Phase Three

Skill Application & Integration

As your confidence grows, the focus shifts from learning skills to applying them. You'll practice new coping strategies in real-life situations — managing stress at home, navigating relationships, and building healthy routines. Sober activities like nature-based therapy, hiking, and community events help you discover that life in recovery can be genuinely fulfilling. Family involvement deepens through therapy sessions and monthly support groups.

Phase Four

Transition & Step-Down

Before you leave PHP, we build a bridge to what comes next. Your care team coordinates your transition to IOP (morning or evening tracks available), connects you with our alumni network, and sets up any external referrals you may need. If sober living is part of your plan, we help arrange that too. You'll leave with a clear aftercare plan, ongoing support, and the knowledge that Trailhead's community remains open to you.

PHP Program FAQs
The average PHP stay is 20–30 days, but treatment length is individualized based on your progress, clinical needs, and insurance coverage. Your care team will work with you to determine the right timeline.
PHP runs 9am–3:30pm, which makes traditional daytime work difficult. However, if you need to work, our IOP has an evening track (6–9pm, Mon–Thu) designed specifically for working professionals. Many clients transition from PHP to the evening IOP when they're ready to return to work.
Most clients use insurance to cover PHP. PHP is more intensive: 9am–3:30pm (full day) vs. IOP's 3.5-hour sessions. PHP includes daily lunch, more group time, and faster clinical access. Most clients start at PHP and step down to IOP as they stabilize.
Absolutely. We offer individual family therapy sessions coordinated with your primary therapist, plus monthly family support groups via Zoom. These cover codependency, family systems, and substance use education — open to any family member 18+, including families of past clients.
Most clients step down to IOP (available in morning or evening sessions). We also connect you with our alumni program, set up external referrals as needed, and help arrange sober living if that's part of your plan. You'll never leave without a clear path forward.
PHP is our most intensive outpatient program. It's appropriate for adults (18–80) coming from detox, those actively using who need daily structure, or anyone who needs more support than IOP provides. We treat both substance use disorders and co-occurring mental health conditions.

Substance Use Disorders

Our PHP treats a full range of substance use disorders including alcohol, opioids, heroin, fentanyl, cocaine, methamphetamine, benzodiazepines, marijuana, kratom, and prescription drug abuse. We also address less common dependencies including synthetic drugs, hallucinogens, inhalants, stimulants, barbiturates, and MDMA. Co-Occurring Mental Health
